The Next 20 Years in Health Care Quality Improvement

A special series from the Journal of the American Medical Association in honor of the 20th anniversary of To Err Is Human and Crossing the Quality Chasm.

More than two decades ago, the Institute of Medicine (now National Academy of Medicine) launched the Quality Chasm Series, setting the agenda for the modern health care quality movement. Although much has been accomplished, serious gaps in quality remain — brought into stark clarity by the COVID-19 pandemic.
