The following initiative, aimed at providing nursing homes a comprehensive tool for developing an evidence-based action plan, occurs in three continuous phases: 1) assessment, 2) planning, training, and implementation, and 3) sustainment. Keys to success at each phase include involvement of the right people, use of information-driven decision making, and careful planning before acting. Some key actions include teamwork coaching, and reinforcement and rewards.
