This study explores failure in communication between health professionals that result in unintended patient harm in a framework of educational, psychological and organizational challenges to the development of effective healthcare teams. To remedy the problems resulting from their errors in communication, the study proposes a seven-step plan including: overcoming educational barriers, training teams together, training teams using simulation, overcoming psychological barriers, creating democratic teams, overcoming organizational barriers, and developing an organizational culture supporting healthcare teams.
