CKD as a Model for Improving Chronic Disease Care through Electronic Health Records

The following paper discusses the strategies and provides general goals for appropriate incorporation of chronic kidney disease (CKD)- related data into electronic health records and corresponding design features that may facilitate optimal care of patients and clinical quality improvements. At the individual patient level, CKD care could be improved by using patient- and provider-facing flow sheets; at the population level, aggregated data could facilitate population quality improvement efforts.
